Long Term Care (LTC Placement)

 

The RUHS Long Term Care (LTC) Program operates under the auspices of the RUHS Public Guardian Administration and serves Conservatees with serious mental illness who require hospitalization or our-of-home placement.

The LTC clinicians, case managers, and peers provide case management, group therapy, and discharge planning services for Conservatees placed at psychiatric hospitals, IMDs, and residential care facilities (also known as board and care facilities).

The LTC staff collaborates closely with the Public Guardian - LPS program. In addition, the LTC staff coordinates their case management services with the consumer's LPS Conservator. Together, these staff members help the Conservatees navigate the various levels of care, from acute inpatient hospitalization to long-term facilities and eventually to community-based residential placements or homes.
