The Cardiovascular Health Innovation (CVHI) is a community-driven effort aimed at improving heart health in Riverside County.

Our mission: 

  • Improve blood pressure control.
  • Reduce disparities in managing hypertension and high cholesterol.
  • Increase access to social support. 

Through a robust learning collaborative, we bring together healthcare providers, clinics and community organizations to track patient progress, strengthen team-based care, and connect clinical and community services for better long-term outcomes. 

With funding through the California Department of Public Health and by the Centers for Disease Control and Prevention, our work targets adults 18+ living within seven local census tracts that are experiencing the highest estimated rates of hypertension (53% or higher) in Beaumont, Desert Palm, Hemet and Menifee.
